What was actually broken.
Care coordinators across a forty-clinic network were reconciling referrals between seven systems, three shared inboxes and a whiteboard. Patients fell through the gaps at every hand-off, and nobody could answer a simple question: where is this referral right now?
How we went at it.
Eleven days of shadowing coordinators came before a line of code. The pattern was clear — the work was not data entry, it was chasing state. So we designed around a single referral timeline: every event, every owner, every overdue item visible in one place, with a keyboard-first queue for the high-volume users and FHIR integration pulling clinical context in rather than asking staff to re-key it.
Where it landed.
Referral leakage dropped sharply in the first quarter after rollout. Coordinators who had been working an hour of unpaid overtime most days stopped doing so, which turned out to be the metric leadership cared about most.
They spent nearly two weeks sitting with our coordinators before proposing anything. Every vendor before them arrived with a demo on day one.
